How Much Time Does Surrogacy Really Take? What to Expect From Start to Finish

Resources for Surrogates

If you’ve been thinking about becoming a surrogate, you’ve probably asked yourself an important question: How much time does surrogacy actually take?

You may be balancing work, raising children, caring for family members, or simply trying to understand whether this journey fits into your life right now. While surrogacy is often described as a pregnancy journey, the reality is that it begins long before embryo transfer and continues after delivery.

Understanding the full surrogacy time commitment can help you make an informed decision, set realistic expectations, and move forward with confidence.

The Short Answer: How Long Does Surrogacy Take?

Every journey is unique, but most surrogate journeys take approximately 14 to 24 months from initial application to postpartum recovery. Several factors can influence the timeline, including:

  • How quickly you complete the screening process.
  • Matching timelines with intended parents.
  • IVF scheduling and clinic availability.
  • Whether the first embryo transfer is successful.
  • The natural timing of pregnancy and recovery.

While that timeframe may sound long, much of it involves waiting periods, preparation, and moments where life continues as usual.

Phase 1: The Application and Screening Process

Estimated timeline: 2 to 3 weeks

The journey typically begins with an application and a series of screenings designed to ensure that surrogacy is a safe and positive experience for everyone involved. During this phase, you can expect:

  • Initial conversations with the agency.
  • Review of your medical records.
  • Background checks.
  • Discussions about your support system.
  • Home assessment.

These steps help determine whether surrogacy is the right fit and allow everyone to move forward confidently. For many women, this stage brings a mix of excitement and reflection as they begin imagining what the journey could look like.

Phase 2: Matching With Intended Parents

Estimated timeline: 1 to 2 weeks

Once approved, you’ll be matched with intended parents whose expectations, communication style, and values align with yours. This phase includes:

  • Reviewing intended parent profiles.
  • Participating in an introductory match meeting.
  • Discussing expectations and preferences.
  • Determining whether everyone feels comfortable moving forward.

Some matches happen quickly, while others take more time. There is no “right” timeline. At GRACE, we encourage thoughtful matching because compatibility often contributes to a more rewarding experience for everyone involved.

Phase 3: Medical clearance

Estimated timeline: 4 to 8 weeks

Once a match has been approved and officially confirmed between the surrogate and the intended parents, the next step is obtaining medical clearance from the IVF clinic selected by the parents. At this stage, your medical records are submitted to the clinic for review.

This initial review typically takes approximately four to five weeks, depending on the clinic’s schedule and workload. If the clinic approves your medical records, an in-person medical screening appointment will be scheduled. This appointment generally includes:

  • A comprehensive medical evaluation, blood work
  • A urine testing
  • An ultrasound
  • Any additional assessments required by the fertility clinic.

Following the appointment, it usually takes about two weeks for all test results to be reviewed and finalized. Once the surrogate receives medical clearance, the team can move forward to the next phase of the surrogacy journey with confidence that all medical requirements have been met.

Phase 4: Legal Contracts

Estimated timeline: 4 to 6 weeks

This stage generally involves reviewing and finalizing legal agreements that outline the rights, responsibilities, and expectations of all parties involved.

Phase 5: IVF, Medical preparation and Embryo Transfer

Estimated timeline: 4 to 8 weeks

Depending on your specific treatment protocol, this period may include fertility medications, monitoring appointments, and coordination with the IVF clinic. Although there can be paperwork and scheduling involved, many surrogates appreciate that this phase provides clarity before pregnancy begins.

The IVF process is one of the more appointment-heavy periods of the journey. You’ll typically begin medications several weeks before transfer to prepare your body for pregnancy. During this time, you’ll attend monitoring appointments to ensure everything is progressing appropriately.

The embryo transfer itself is relatively quick, but emotionally, it can feel significant. Following the transfer, there is often a waiting period before pregnancy can be confirmed.

It’s important to remember that not every transfer results in pregnancy. Some journeys require additional transfers, which can extend the overall timeline.

Phase 6: Pregnancy

Estimated timeline: Approximately 40 weeks

Once pregnancy is confirmed, life often settles into a more familiar rhythm. You’ll attend regular prenatal appointments, maintain communication with the intended parents, and continue balancing your daily responsibilities.

For many surrogates, this phase includes:

  • Routine prenatal care.
  • Periodic communication with intended parents.
  • Preparing for delivery.
  • Navigating the normal joys and challenges of pregnancy.

Although pregnancy lasts around nine months, every experience is different. Some women continue working throughout much of the pregnancy, while others may need additional flexibility depending on their health and circumstances.

Phase 7: Delivery Day

Estimated timeline: One unforgettable moment

Delivery day often arrives with a mixture of excitement, anticipation, pride, and emotion. Hospital plans are usually discussed well in advance, including:

  • Who will be present during labor and delivery.
  • How the intended parents will be involved.
  • Immediate postpartum expectations.
  • Legal documentation and hospital coordination.

While each birth story is unique, many surrogates describe witnessing intended parents meet their baby for the first time as one of the most meaningful moments of the journey.

Phase 8: Postpartum Recovery

Estimated timeline: 6 to 12 weeks

Even after delivery, your journey deserves care and attention. Recovery looks different for everyone. Physical healing, hormonal adjustments, emotional processing, and returning to everyday routines all take time.

During this period, you may focus on:

  • Postpartum medical appointments.
  • Rest and recovery.
  • Emotional well-being.
  • Reconnecting with work and family routines.
  • Determining what ongoing communication with the intended parents may look like.

Allowing yourself the space to recover is an important part of honoring everything your body and heart have experienced.

Is the Time Commitment Worth It?

Only you can answer that question. Surrogacy asks a great deal of you. It requires time, patience, flexibility, and emotional investment.

But many women say that despite the appointments, paperwork, and occasional unpredictability, the experience became one of the most meaningful chapters of their lives.

They often describe the journey not in terms of what they gave up, but what they gained: lifelong friendships, personal growth, a deeper appreciation for their own strength, and the opportunity to help another family experience parenthood.
